Radiofrequency Lesioning

What is it?
Radiofrequency lesioning is performed after the facet joints have been tested and confirmed (see diagnostic block). If these small branches of nerves called the medial articular branch are “turned off” through this procedure the facet joint in question can be made pain free.

The Lesioning
This procedure is performed under the guidance of x-ray to provide proper placement of the RF (radiofrequency) probe. Intravenous medication will be delivered to provide you with some sedation throughout the procedure. Depending on your pain symptoms and the results of the facet joint mapping procedure, the RF probe may be placed at more than one level or on both sides. You will feel a vibration in the area of your discomfort which will indicate that the RF probe is positioned properly. Further confirmation of the probe location will be made by motor testing; during which time you will feel a thumping or a twitching of the muscles in your low back or neck region. Once placement is confirmed the nerve will be “heated or lesioned” to 80 degrees for approximately 90 seconds at each level being treated. This whole process takes about 30-45 minutes; however, afterwards you will be monitored for approximately 30 minutes by the nurses before going home.

After the injection
There are not too many restrictions following this procedure; however, you should take it easy the first day. You will have tenderness in the injection areas for a few days following this procedure. Applying ice to the injected areas will help reduce any swelling and/ or irritation caused from the injection. There is a 60% chance of pain relief following this procedure. This typically lasts approximately 3 months to 1 ½ years. The nerve eventually grows back and the procedure can be repeated. While you are experiencing pain relief, vigorous physical therapy is necessary to try and strengthen the involved facet joints. Following up with your physician 1-2 weeks following the procedure is recommended to assist in your treatment plan.
